Circuit Court sustained accidental injuries on April 2, 2004. The accident occurred when the Petitioner
was descending a ladder while holding files, when he slipped on a plastic
cover and fell to the ground, and as a result he injured his lower back and
buttocks (L5-S1 herniated disc with radiculopathy requiring a spinal fusion
at L5-S1). On October 16, 2006, the
Arbitrator awarded the Petitioner $58,310.08.
The Finance Subcommittee on Workers’ Compensation approved this
decision on December 7, 2006. However,
this amount represents an underpayment of $562.66 in TTD benefits. Interest on this amount is $6.63. Therefore, the Petitioner is due an
additional $569.29 as a result of the underpayment. This additional settlement is within the
grant of authority conveyed by the Cook County Board of Commissioners to the
State’s Attorney’s Office. State's

Arbitrator, Award No. 04-WC-17024 in the amount of $569.29 and recommends its
